        NEW 38 HOUR COMMERCIAL DRIVING SCHOOL

 

Requirements for Owners and Instructors

  1. Must be at least 21 years of age.
  2. Must hold a valid driver’s license.  The license must be a class D Chauffer’s License if the instructor will be administering behind-the-wheel instruction for the 38 hour course.
  3. Must have at least a High School Diploma or GED.
  4. Must be a Louisiana resident.
  5. Must pass a background check.
  6. Must submit a complete application package as outlined below.

 

Application Instructions - The internet applications can be completed on- line with the exception of the signature and notary portion.  The applications can also be handwritten.

  1. Potential Commercial Driving School must complete the following:

a.       Read and understand LA Administrative Code, Title 55, Part III, Chapter 1.  Submit a notarized Rules and Regulations Commonly Abused form from each owner and or instructor.

b.      Submit a completed 38 Hour Commercial Driving School Application, along with a $25 certification fee.  Note:  E-mail address required.

c.       Submit a completed Commercial Driving School Instructor Application along with a $10 certification fee for each instructor.

d.      Submit completed Background Check Forms for each owner and/or instructor.  Each person must complete the personal information on the forms.  See attached instructions and forms.  A separate $26 fee must be submitted for each background check.    

e.       Submit a copy of a Course Guide. (Refer to the attached Rules & Regulation Document,)

f.       Submit a copy of a Program of Instruction/Course Content.  (Refer to the attached Rules & Regulation Document)

g.      Submit a copy of final tests.

h.      Submit a copy of driver education certification for each owner and instructor.

i.        Submit a photocopy of the driver’s license of each owner and instructor.

j.        Submit a certificate of insurance which will include description of vehicle, VIN, effective dates and limits of liability (25/50/10).  This certificate is submitted by the insurance company and must have Office of Motor Vehicle, Attention:  Debbie Hoover, P. O. Box 64886, Baton Rouge, LA 70896 as the certificate holder.  This certificate must ensure this agency will be notified of any cancellation.

k.      If physical location is a non-classroom facility, you must file for approval a Planned Review Application to the State Fire Marshal’s Office.  The notice of approval must be submitted to this office.

  1. The application package must be complete.  Partial application package will not be accepted and mailed back for completion.  All fees must be submitted in the form of a money order, cashier’s check or            business check made payable to Department of Public Safety.  All the above documents with original      signatures must be mailed to:

  1. After the completed application package is received and the owner has passed the background check, copies of the Certificate of Successful Completion will be sent to you and a sample of a completed certificate must be sent back to this office for approval.
  2. The Office of Motor Vehicles may meet with the potential Commercial Driving School Owner.
